Effective July 29, 2013 - new telephone number for Front Door referrals:
What is a Front Door Contact and Referral?
1) Individual new to OPWDD services
2) Individual with an approved eligibility determination who has never received OPWDD services or who has not been enrolled in active OPWDD services for an extended period of time
3) OPWDD eligible individual that receives only MSC and / or FSS services
All contact and referrals to the Front Door must be made to the new telephone number.
Effective immediately, front door referrals will no longer be received by Bonnie Evert via fax, emails or telephone messages.
Front Door calls will be answered Monday - Friday between the hours of 8:30am to 12:00 noon and 12:30pm to 4:00pm.
To facilitate services as efficiently as possible, it is preferred that calls are received during the business hours noted above. However, you may leave a voice message at any time and your call will be returned as soon as possible.
