Certificate of Business Name - Doing Business As (DBA)


If an assumed business name is to be used, or if the business is to be a partnership, section 130 of the general business law requires the filing of a business certificate with the county clerk in the legal supply houses. It is, of course, advisable to consult with an attorney regarding the preparation of this or any other legal document.

  1. Purchase four (4) Business Certificates for a Sole Proprietorship or Partnership at a business stationary store.
  2. Complete the top portion and the backside of the certificates (You may type, print, or write legibly.) Have certificates signed by a Notary Public, Commissioner of Deeds, or an Attorney. (A Notary Public can be found at any bank)
    1. File the four notarized business certificates.
    2. Clerks' office keeps original
    3. Bank keeps second certificate for business account
    4. Certificate is/may be displayed at your place of business
    5. Certificate is for special license of permit (if necessary)
  3. You may file in person or by mailing the necessary certificate along with a Check or money order payable to and send to:
