Residence Requirements:

THERE ARE NO RESIDENCY REQUIREMENTS FOR PARTICIPATION IN THIS EXAMINATION. There is one anticipated vacancy at the Erie County Medical Center Corporation.

Examples of Duties:

An Assistant Nurse Case Manager  collaborating with physicians and other clinical staff to develop and coordinate the plan of care for a specific caseload of patients;

Provides comprehensive management of each patient's transition from admission through discharge;

Collaborates with physicians and interdisciplinary team to develop and implement best practice models and standards for each individual patient and for specific diagnoses;

Develops, implements and monitors the patient's interdisciplinary plan of care; revises goals and plans as needed based on changes to the patient's clinical condition;

Individualizes care coordination and discharge plans based on the patient's social and economic situation, family resources, functional abilities, knowledge needs, potential risk factors and complications and special concerns or issues;

Assesses and evaluates each patient's plan of care on a daily basis;

Communicates with third party payers to provide initial and concurrent reviews for authorization of hospital stay;

Coordinates the discharge plan within the interdisciplinary team to assure the availability of resources in support of safe and timely discharge and delegates appropriate actions and tasks to team members;

Identifies barriers to care coordination, discharge and potential avoidable hospital days; reports and discusses barriers with departmental leadership and/or Physician Advisors as needed to facilitate resolution and expedite appropriate care;

Collaborates and communicates with key members of each patient's team, involved parties in pursuing desired patient outcomes and discharge plan and with patient/family about progress toward expected outcomes and satisfaction with care delivered;

Completes assessment tools as required; Consults with appropriate parties to balance care and cost according to internal and external data sources; Attends and participates in interdisciplinary meetings, mandatory and reorientation programs, and departmental Performance Improvement activities

Qualifications:

MINIMUM QUALIFICATIONS:  Candidates must meet one of the following requirements on or before the date of the written test: 

A) Graduation from a regionally accredited or New York State registered college or university with a Bachelor's Degree in Nursing, Health Administration, Human Services or a closely related field and two (2) years of behavioral health and/or medical/surgical registered professional nursing experience; or:

B) Graduation from a regionally accredited or New York State registered college or university with an Associate's Degree in Nursing, Health Administration, Human Services or a closely related field and four (4) years of behavioral health and/or medical/surgical registered professional nursing experience; or:

C) An equivalent combination of training and experience as set by the limits of (A) and (B).

SPECIAL REQUIREMENTS:

1. Possession of a license and current registration to practice as a Registered Professional Nurse as issued by the New York State Department of Education; and:

2. Possession of a Certification as a Patient Review Instrument (PRI) Assessor as issued by the New York State Department of Health by completion of the probationary period.

1. Verifiable part-time and/or volunteer experience will be pro-rated toward meeting the experience requirements. 

2. Your degree and/or college credit must have been awarded by a regionally accredited college or university or one recognized by the New York State Education Department as following acceptable educational practices. A grade of "D" or better is necessary for a course to be credited as successfully completed. If your degree and/or college credit was awarded by an educational institution outside of the United States and its territories, you must provide independent verification of equivalency. You can write to this Department for a list of acceptable companies providing this service; you must pay the required evaluation fee.

Subjects of Examination:

Scopes / Subjects of examination: A test designed to evaluate knowledge, skills and /or abilities in the following areas.

Preparing written material

These questions test for the ability to present information clearly and accurately, and to organize paragraphs logically and comprehensibly. For some questions, you will be given information in two or three sentences followed by four restatements of the information. You must then choose the best version. For other questions, you will be given paragraphs with their sentences out of order. You must then choose, from four suggestions, the best order for the sentences.

Principles and practices of social casework

These questions test for knowledge of the principles and practices used to provide casework services to individuals and/or families of all ages, backgrounds, and circumstances. Questions will test for application of this knowledge in various settings and will include such topics as assessment, monitoring and evaluating services, counseling, referrals, and advocacy.

Working with individuals to promote mental health

These questions test for knowledge of characteristics, causes and treatment methods associated with working with individuals who may have mental, developmental, physical, social and substance abuse disorders and/or disabilities. Questions cover such topics as establishing, maintaining and terminating client relationships; recognizing, interpreting, and responding to individual and group behaviors, as well as crisis situations; and facilitating access to essential services such as financial, housing, medical, educational, legal, vocational, and recreational.

Interviewing

These questions test for knowledge of the principles and practices employed in obtaining information from individuals through structured conversations. These questions require you to apply the principles, practices, and techniques of effective interviewing to hypothetical interviewing situations. Included are questions that present a problem arising from an interviewing situation, and you must choose the most appropriate course of action to take.

NOTICE TO CANDIDATES: Unless otherwise noted, candidates are permitted to use quiet, hand held, solar or battery powered calculators. Devices with typewriter keyboards, "Spell Checkers", “Personal Digital Assistants", "Address Books", "Language Translators", "Dictionaries", or any similar devices are prohibited. You may not bring books or other reference materials.
