The Erie County Department of Health is currently working with community partners to gather information from residents to help with public health planning for the next four years. Your responses will help us identify gaps in services and needs in the community.  We will then use this information to create a plan to fill gaps and address the county's needs going forward. CHA and Improvement Planning

The Community Health Assessment (CHA) provides information and help measure how well a public health system is fulfilling functions.  A CHA is part of an ongoing broader community health improvement process.  This process uses CHA data to identify priority issues, develop and implement strategies for action, and establish accountability to ensure measurable health improvement, which are outlined in the Community Health Improvement Plan (CHIP). A community health improvement process involves collaborative participation by the  local health department, organizations,  health care providers, health insurance providers, community based organizations, advocacy groups, academia, employers, state agencies and businesses whose activities can influence the health of individuals and communities.
