Single Point of Access (SPOA)

The goal of Adult SPOA is to have each person linked with the appropriate services to achieve successful outcomes.

An integrated SPOA (care management, ACT, AOT and housing) improves access, fosters early engagement, and provides priority services to individuals with high service needs. This includes individuals who are transitioning from institutional settings (i.e. psychiatric hospitalization or incarceration) back into the community and individuals that have a high risk of future or a history of deep end, high cost system utilization (i.e. psychiatric or medical ER visits or hospitalization, homelessness, or incarceration).

The Adult Single Point of Access (A-SPOA) provides access to intensive community-based services, including:

Care Management, ACT, and/or Housing services are intended for individuals who are at high risk of further system utilization. These services are for individuals who are unable to maintain community-based linkages and important supports. Services will link clients to psychiatric treatment, medication management, medical providers, housing, substance abuse treatment, financial services, social supports, and legal representation.

Eligible applicants must have a psychiatric diagnosis from the Diagnostic Statistical Manual (DSM) as diagnosed by a licensed mental health professional considered a Serious Mental Illness (SMI), such as but not limited to:

  • Schizophrenia Disorder
  • Schizoaffective Disorder
  • Delusional Disorder
  • Psychotic Disorder
  • Bipolar Disorder
  • Major Depressive Disorder
  • Post-Traumatic Stress Disorder (PTSD)

Making A SPOA Referral

To make a referral, go to www.eriespoa.org and select Submit a Referral at the top of the webpage.

A signed Erie County SPOA Consent Form must be included with the referral to be considered.
