Forensic Mental Health Community Discharge Planner to provide effort toward completion of community discharge planning activity to incarcerated individuals receiving mental health services.  Must have good knowledge of human behavior, mental health, co-occurring disorders, and substance use/addictions treatment services.  As well as skills in interviewing, as well as both oral and written communication.  Knowledge surrounding interface between legal, penal, and behavioral health systems a plus.  Must hold a minimum of Bachelor’s Degree in Social Work, Psychology, Criminology, or Nursing.  Min 1 yr exp providing treatment, case management, or rehabilitation services to individuals with Serious Mental Health Disorders.  Competitive salary.
