Everybody Has Needs - Do the Right People Know What Yours Are?

If you or someone in your household has a disability or a special medical need, those you call for help in an emergency need to know. Seconds can make a life-or-death difference, and having specific details about your special situation will significantly help emergency responders help you.

The process is simple:

  1. Residents of Erie County fill out the Functional Needs Registry Form. This is strictly voluntary and the data will be kept strictly confidential. It will be available only to local emergency assistance officials. Please print clearly and provide all information.

  3. The form will be forwarded to the appropriate Emergency Manager within the appropriate town, village, or municipality for their confidential database.

If another October Storm, November 2015 Storm (Winter Storm Knife). or any other disaster were to occur, each municipality would know who to check on and assist immediately.
