Grants of Assistance for Guide Dogs

• Emergency Services, 460 Main St, 2nd floor, may assist you with ongoing cash benefits for the purchase of dog food, if: (1) you receive SSI, (2) are deaf, blind, or disabled, and (3) have a registered guide, hearing, or service dog.   

• Along with other common documents needed for application, applicants must complete an LDSS-3087, and submit proof of your dog’s guide/hearing/service registration.
