Residence Requirements:

THERE ARE NO RESIDENCY REQUIREMENTS FOR PARTICIPATION IN THIS EXAMINATION. There is one anticipated vacancy at the Erie County Medical Center Corporation. 

Examples of Duties:

Population Health Services Coordinator coordinates population health work activities and initiatives necessary to improve patient health status and meet established organizational performance benchmarks;

Coordinates and may perform work activities associated with population health initiatives typically performed by lower-level staff, such as scheduling appointments, follow-up phone calls and various post-discharge care activities, designed to motivate patients to attend regular health screenings and improve the health status of specific patient populations;

Assists department administration and clinical staff in developing and evaluating population health patient initiative programs necessary to enhance patient participation and eliminate gaps in care coordination;

Utilizes data resources, including electronic health records (EHR), to monitor patients’ participation in such initiatives against benchmarks established by insurance providers and collaborative organizations;

Develops data reports and communicates patient participation information and benchmark status with departmental staff and collaborative organization membership;

Identifies work priorities, prioritizes work assignments and assigns such work to lower-level staff;

Assists licensed staff by making referrals and performing related work as needed for patients requiring comprehensive case management;

Meets with and presents to community organizations, healthcare providers, insurance providers and the general public to enhance the awareness of population health initiatives performed at ECMCC;

Reviews patient medical charts and records for presence of advanced directives; communicates findings with appropriate personnel;

Reviews reports and other documents for completeness, accuracy and conformity with established procedures; Organizes and participates in meetings, conferences, in-services, trainings, etc.

Qualifications:

MINIMUM QUALIFICATIONS:  Candidates must meet one of the following requirements on or before the date of the written test:

A.) Graduation from a regionally accredited or New York State registered college or university with a Master’s Degree and two (2) years of experience in providing, arranging or facilitating patient discharge services or patient post-discharge services or two (2) years of experience as a licensed practical nurse or professional registered nurse; of which one (1) year was in an ambulatory services setting*; or:

B.) Graduation from a regionally accredited or New York State registered college or university with a Bachelor’s Degree and four (4) years of experience in providing, arranging or facilitating patient discharge services or patient post-discharge services or four (4) years of experience as a licensed practical nurse or professional registered nurse; of which one (1) year was in an ambulatory services setting*; or:

C.) Completion of sixty (60) semester credit hours at a regionally accredited or New York State registered college or university and six (6) years of experience in providing, arranging or facilitating patient discharge services or patient post-discharge services or six (6) years of experience as a licensed practical nurse or professional registered nurse; of which one (1) year was in an ambulatory services setting*; or:

D.) Graduation from high school or possession of a high school equivalency diploma and eight (8) years of experience in providing, arranging or facilitating patient discharge services or patient post-discharge services or eight (8) years of experience as a licensed practical nurse or professional registered nurse; of which one (1) year was in an ambulatory services setting*; or:

E.) An equivalent combination of training and experience as set by the limits of (A), (B), (C) and (D.)

Notes:

1. *Ambulatory services is defined, for qualifying purposes, as medical care in which patients are admitted, treated and discharged all on an outpatient basis. Examples include outpatient community-based practices, hospital clinics, outpatient dialysis, emergency room, outpatient rehabilitation services, infusion, dentistry and ambulatory surgery centers.

2. Verifiable part-time and/or volunteer experience will be pro-rated toward meeting the experience requirements.

3. Your degree and/or college credit must have been awarded by a regionally accredited college or university or one recognized by the New York State Education Department as following acceptable educational practices. A grade of "D" or better is necessary for a course to be credited as successfully completed. If your degree and/or college credit was awarded by an educational institution outside of the United States and its territories, you must provide independent verification of equivalency. You can write to this Department for a list of acceptable companies providing this service; you must pay the required evaluation fee.

Subjects of Examination:

Scopes / Subjects of examination: A test designed to evaluate knowledge, skills and /or abilities in the following areas.

Preparing written material
These questions test for the ability to present information clearly and accurately, and to organize paragraphs logically and comprehensibly. For some questions, you will be given information in two or three sentences followed by four restatements of the information. You must then choose the best version. For other questions, you will be given paragraphs with their sentences out of order. You must then choose, from four suggestions, the best order for the sentences.

Medical terminology
These questions test for knowledge of words, phrases, and abbreviations used in medical diagnosis and therapy. Questions may cover such topics as diseases, symptoms, pharmaceuticals, medical specialties, anatomy, treatment modalities, medical supplies, and medical equipment. Prefixes and suffixes used in medical terminology may also be included.

Supervision
These questions test for knowledge of the principles and practices employed in planning, organizing, and controlling the activities of a work unit toward predetermined objectives. The concepts covered, usually in a situational question format, include such topics as assigning and reviewing work; evaluating performance; maintaining work standards; motivating and developing subordinates; implementing procedural change; increasing efficiency; and dealing with problems of absenteeism, morale, and discipline.

Understanding and interpreting written material
These questions test for the ability to understand and interpret written material. You will be presented with brief reading passages and will be asked questions about the passages. You
should base your answers to the questions only on what is presented in the passages and not on what you may happen to know about the topic.

Devices with typewriter keyboards, "Spell Checkers", “Personal Digital Assistants", "Address Books", "Language Translators", "Dictionaries", or any similar devices are prohibited. You may not bring books or other reference materials.
