"Vax Visit" - Vaccination in Your Home 

  • Call (716) 858-2929 to make an appointment to be vaccinated at your home.  This service is free and available to Erie County residents ages 12 and older.  

Pfizer & Moderna Vaccination Sites (Dose 1, 2 and 3)

  • Available at the locations listed below.  Locations are updated regularly.  
  • For Dose 1 & 2 appointments are recommended (register below) and walk-ins are accepted. Dose 3 is walk-in only. 
  • Pfizer is approved for ages 12 and older & Moderna for ages 18 and older. See COVID-19 Consent for Vaccination of Minors for those under age 18.
  • Persons should not receive the second dose of Pfizer vaccine earlier than the recommended 21 days after the first dose. Persons should not receive the second dose of Moderna vaccine earlier than the recommended 28 days after the first dose.  
  • Vaccine types should not be mixed. 
Special Notes and Instructions for Dose 3 of Pfizer or Moderna
            • Check the qualifications to see if you qualify and what documentation you need to bring with you.  If you have additional questions about this, please contact your healthcare provider.
            • Complete and bring the COVID-19 Vaccine Third Dose Attestation Form with you. Forms will also be available at the clinic locations.  Remember to bring a letter from your healthcare provider if you have a medical condition that is not included in the current qualifications. Those with a condition that is included in the current qualifications, do not need to bring a letter from their healthcare provider.

After Vaccination Information

  • If you believe that you had an adverse reaction to any COVID-19 vaccine: 
          • Contact your healthcare provider.
          • Report a vaccine reaction by completing the Vaccine Adverse Event Reporting System Online Form (VAERS) or the downloadable PDF. 
          • Important: If you are experiencing a medical emergency, seek immediate assistance from a healthcare provider or call 9-1-1. If you need individual medical or health care advice, consult a qualified healthcare provider. 
  • Participate in v-safe, a smartphone-based tool that checks in on you after your COVID-19 vaccination. 
          • Your participation helps keep COVID-19 vaccines safe for you and for everyone.
          • If you got vaccinated in the last 6 weeks, you can participate in v-safe!
