ERIE COUNTY OFFICE OF HEALTH EQUITY DIRECTOR TO JOIN HEALTHeLINK REGIONAL HEALTH IMPROVEMENT COMMITTEE

ERIE COUNTY, NY – Kelly Wofford, director of the Erie County Department of Health (ECDOH) Office of Health Equity, has been invited to join the Regional Health Improvement Committee for HEALTHeLINK.

“HEALTHeLINK will benefit from Kelly’s disciplined understanding of how social determinants of health affect health outcomes and quality of life,” said Commissioner of Health Dr. Gale Burstein. “Her experience within Erie County’s health care infrastructure will be a great asset to HEALTHeLINK as they develop strategies to improve patient care and reduce health disparities.”

“Establishing the regional health improvement committee is part of our affiliation agreement with Population Health Collaborative and will help to guide the continuation of Live Well Western New York in addition to informing our enhanced focus on assisting efforts to increase health equity and address social determinants of health utilizing health information technology,” said HEALTHeLINK Executive Director Dan Porreca. “Kelly will add her experience and knowledge in addressing health equity taking into account social determinants and we welcome her to the new committee.”

Among other priorities, this committee focuses on population health initiatives related to health equity, prevention and chronic condition management, and connects HEALTHeLINK with community-based organizations, government agencies, business stakeholders and others in the eight counties of Western New York.
