We recommend you discuss vaccinations with, and receive them from, your primary care provider.  If your primary care provider cannot meet your vaccination needs, or you do not have a primary care provider, please call to speak with a staff member to determine if we can assist you.

Age Requirement

  • 5 years and older.


  • By appointment on the 2nd and 4th Fridays of every month from 8:30 am to 11:30 am.  Please call for available dates, as holidays may impact when clinic dates are cancelled/moved to other weeks.


  • Visit fee of $20.00 plus the cost of the vaccine if applicable (see chart below)  
  • Payment is due at the time of your visit. 
  • We do not accept insurance.
  • Payment methods accepted:
    • Cash (exact change)
    • Credit card (Visa, Master Card, Discover, American Express – nominal fee applied to all)
    • Check 

Please Bring with You

  • All immunization records
  • Documentation from school showing requirement that student needs to be immunized (when applicable)
  • Parent or legal guardian for those under age 18

Uninsured or under-Insured school children, who are identified by local school authorities, to be excluded from school for lack of acceptable evidence of immunization
