Residents with an issue will be asked to include their name, address and contact information.The form also includes a list of options for "Nature of Complaint," they are:
- Bridge
- Permit
- Snow Removal
- Potholes
- Vehicle Damage
- Property Damage
- Mailboxes
- Signage
- Striping
- Pipe/Ditching
- Deer
The complainant will also be asked to provide as much information as possible explaining the complaint and the specific location of their complaint.
